• Download and fill out our application form and  fax or email to us with your one time registration fee of $50.00. 
• Include on your application form your Canadian business number with your GST# and PST# if applicable.
• Sign agreement form to the assigned credit card number on the application form (we are currently not charging the monthly membership fee of $15.00 to keep your membership active and can be cancelled at any time with 30 days written notice).
• Payment due at time of order using your Visa, MasterCard or American Express card. We charge you your discounted rate on the club baskets. You will place your order with us directly by phone, fax, email or on the www.expressgiftbaskets.net website using your company identification number.
